Nursery School Registration Form 2022-2023

To submit your child’s registration for the Holy Spirit Christian Nursery School you should:
  1. Complete and submit the form below on-line, or click on the print button below to print out a blank form and complete manually..
    2. Remit the Required $75.00 registration fee. You can do that in any of three ways. After completing the form you can click on the button below the form to make a secure payment through our on-line provider. Alternatively you can use the church’s kiosk, located down the hall from the nursery classrooms in the lobby of the main Worship Center. Finally, you can choose to hand in a check to the nursery school director or the church office.
  2. Remit the first month’s tuition before June 15th.
  3. You will be asked by the school director to sign a version of the submitted form as parent or guardian before class begins.

To complete the registration process, the non-refundable fee ($75.00) must accompany the form. One registration fee is required per family. You can pay via check, or through the kiosk in the church nave, or on line after completing this form. Checks should be made payable to Holy Spirit Christian Nursery School. The registration fee holds a spot in a class until June 15th, when the September tuition payment is due. Tuition is a yearly fee and divided into nine payments. The first installment (September tuition) is due on June 15th. This payment will reserve your child's spot in the class. No initial tuition payments (September) are refunded after July 1st..
